Grant of Rights I, the undersigned Subject, hereby grant to the Company, its successors, assigns, licensees, and legal representatives, the absolute and unconditional right and permission to use, reproduce, publish, display, distribute, and create derivative works from my name, image, likeness, voice, and biographical information (collectively, "Likeness") as captured in photographs, video recordings, audio recordings, or other media (collectively, "Materials") taken on or about ___________ at ___________ in connection with the following: **Project/Purpose:** ___________ ## 2.
