As a covered federal contractor, we are required to invite you to voluntarily self-identify your veteran status. This invitation is extended to all applicants and employees. We are committed to affirmative action on behalf of protected veterans and to taking positive steps to recruit, hire, promote, and retain protected veterans in our workforce. --- ## Your Participation Is Voluntary **Completion of this form is voluntary.** You are not required to self-identify your veteran status. Your decision to identify, decline to identify, or not respond: - Will have **no effect** on any hiring, promotion, compensation, or other employment decision. - Will not subject you to any adverse treatment. - Will not be disclosed to your supervisor or other personnel making employment decisions about you. Self-identification data is maintained in a **confidential file separate from your personnel file** and is used only for affirmative action planning, OFCCP reporting, and VETS-4212 federal contractor reporting. --- ## Definitions of Protected Veteran Categories The categories below are defined by VEVRAA and OFCCP regulations. Please read each definition carefully before making your selection. **Disabled Veteran:** A veteran who served on active duty in the U.S. military and is entitled to disability compensation (or who would be entitled to compensation except for the receipt of military retired pay) under laws administered by the Secretary of Veterans Affairs; or a person who was discharged or released from active duty because of a service-connected disability. **Recently Separated Veteran:** A veteran during the three-year period beginning on the date of such veteran's discharge or release from active duty in the U.S. military, ground, naval, or air service. **Active Duty Wartime or Campaign Badge Veteran:** A veteran who served on active duty in the U.S. military, ground, naval, or air service during a war, or in a campaign or expedition for which a campaign badge has been authorized under the laws administered by the Department of Defense. **Armed Forces Service Medal Veteran:** A veteran who, while serving on active duty in the U.S. military, ground, naval, or air service, participated in a United States military operation for which an Armed Forces service medal was awarded pursuant to Executive Order 12985 (61 Fed.
